Today, however, mounting evidence suggests that China’s medical sector is under severe strain.

Hospital bankruptcies, salary cuts, delayed wage payments, shrinking reimbursements, rising debt burdens and growing public dissatisfaction all point to a healthcare system confronting a structural crisis rather than a temporary downturn.

The most alarming development is that even public hospitals, once considered among the safest institutions in China, are now struggling financially. Reports from various provinces indicate that hospitals are delaying salaries, cutting bonuses, reducing staff and, in some cases, suspending operations entirely. According to recent reports, public hospitals across China are facing mounting debt and declining revenues after years of aggressive expansion combined with policy reforms that have reduced traditional income streams. At the heart of this crisis lies a dangerous combination of policy failures and entrenched corruption.

During China’s economic boom years, hospitals expanded aggressively. New campuses were built, expensive medical equipment was purchased, and healthcare institutions competed for higher classifications and prestige. Many of these projects were financed through debt rather than sustainable revenue growth. The consequences are now becoming evident. Public hospital debt nearly quadrupled between 2011 and 2021, according to data cited in multiple reports.

This expansion occurred within a political system where accountability often takes a back seat to bureaucratic targets. Local officials were incentivised to pursue visible infrastructure projects rather than long-term sustainability. Hospitals borrowed heavily, assuming continued economic growth and uninterrupted government support. When China’s property market entered a prolonged downturn and local government revenues weakened, the financial foundation beneath many healthcare institutions began to crack.

China’s healthcare sector has long faced allegations of corruption involving government officials, hospital administrators, pharmaceutical companies and procurement networks.

The problem is not limited to isolated scandals. Rather, it reflects a systemic environment where decisions are often shaped by political incentives rather than patient welfare.

When corruption becomes embedded at the highest levels, it inevitably trickles down through the bureaucracy. Senior officials may manipulate procurement contracts, hospital administrators may pursue prestige projects, and mid-level managers often learn that career advancement depends more on political loyalty than efficient governance.

The result is a system where resources are frequently misallocated while ordinary doctors, nurses and patients pay the price.

China’s volume-based procurement programme, introduced in 2018 to reduce drug costs, has undoubtedly lowered prices for many medicines. However, it has also generated controversy regarding hospital revenues, procurement incentives and concerns about product quality and market distortions.

Another major threat is the growing pressure on China’s medical insurance system.China’s rapidly aging population is increasing healthcare demand while the number of working-age contributors is growing much more slowly. Research published in 2025 highlighted that population aging is placing significant pressure on the financial sustainability of China’s social health insurance funds.

The worker-to-retiree ratio continues to decline, creating a long-term imbalance between contributors and beneficiaries. Recent official data show more than 103 million retirees are now covered under employee medical insurance programmes, while demographic pressures continue to intensify.

This demographic squeeze is compounded by economic weakness. Rising unemployment and declining private-sector confidence reduce contributions into insurance funds at the very moment healthcare costs are climbing. Many analysts warn that unless significant reforms are implemented, reimbursement pressures will continue growing.

A healthcare system depends on predictable cash flow. In China, delayed medical insurance reimbursements have become an increasing challenge for hospitals. When hospitals are forced to wait months for payments, day-to-day operations become far more difficult. Healthcare institutions must still pay staff, purchase medicines, maintain equipment and deliver patient care regardless of whether reimbursements arrive on time. The result is a vicious cycle: financial stress leads to wage delays, staff departures, reduced services and declining patient confidence. Increasing numbers of healthcare workers have reported salary cuts, unpaid wages and disappearing performance bonuses.

Behind every statistic lies a personal tragedy: Nurses are working longer hours for less money, Doctors face rising workloads while seeing income reductions. Contract workers are often the first to be dismissed, some healthcare professionals report waiting months to receive wages they have already earned.

For patients, the experience is equally troubling, medical costs remain high; reimbursement levels for certain treatments have been reduced resulting in families delay seeking care because of financial concerns, elderly patients face increasing difficulty navigating an overcrowded and financially stressed system.

The irony is striking, while China’s healthcare infrastructure expanded dramatically over the past two decades, access to affordable and reliable care is becoming more uncertain for many citizens.

The crisis affecting China’s hospitals are not merely a healthcare story. It is also a reflection of broader economic and political stresses. A shrinking workforce, an aging population, local government debt burdens, declining property revenues and a rigid political system have all converged on the healthcare sector. Hospitals are becoming the visible frontline where these pressures collide.

China still possesses enormous healthcare capacity, vast infrastructure and substantial state resources. Yet unless transparency, accountability and genuine structural reform replace debt-driven expansion and bureaucratic mismanagement, the deterioration may accelerate.

For millions of Chinese citizens, the concern is no longer whether the system needs reform. The concern is whether reform will arrive before the strain becomes irreversible.

(The author, Mg Phyu Phway, is a researcher who studies and monitors Asia-Pacific
